Background

The Philippine Society for Responsible Parenthood, Inc. (PSRP), in partnership with the United Nations Population Fund (UNFPA), is implementing an integrated project to strengthen the Reproductive and Maternal Health (RMH) Service Delivery Network and operationalize telemedicine systems in Laguna Province to accelerate maternal mortality reduction. This collaboration builds on PSRP's extensive experience in reproductive and maternal health programming, service delivery network strengthening, and digital health implementation.

The current project focuses on enhancing coordination across public and private providers, improving maternal risk identification, and integrating telemedicine functionalities within existing platforms to ensure timely, quality, and continuous RMH services. The engagement of an institution is critical to assess the Maternal Health Facility Readiness by identifying the locations of local public-private maternal health facilities and validating their service readiness and maternal care gaps, in alignment with project objectives and UNFPA-supported health systems strengthening efforts.

Objective

The primary objective is to strengthen the maternal health service delivery network (SDN), building a functional, integrated, and accountable public and private network of providers across different levels of care. Specifically, the objectives include:

Geographic inventory: Identify the exact geographic locations and distribution of all public and private maternal health facilities in San Pablo City, along with the Laguna Medical Center, Sta. Cruz.

Facility readiness assessment: Assess the readiness and service capabilities of mapped facilities against national and international standards, UHC reforms, and PhilHealth accreditation requirements through self-assessment and validation.

Facility improvement plan and HCPN arrangement strategy: Develop actionable facility improvement plans and a public-private HCPN arrangement strategy to support a functional, integrated, and accountable maternal health service delivery network.

Scope of Work

Under the technical guidance and supervision of PSRP, the contracted institution is expected to carry out the following:

Public-Private Facility Mapping: A complete inventory of private and public facilities (lying-in centers, birthing homes, and hospitals) in San Pablo City and Santa Cruz.

Tool Development and Validation: Develop and validate the Maternal Health Facility Readiness Self-Assessment Tool through literature review, stakeholder consultations, expert validation, and pilot testing.

Facility Readiness Assessment and Community Feedback: Orient facilities on the self-assessment process, support completion of the assessment, and conduct community consultations with women and adolescent clients to identify root causes of gaps across the continuum of care.

Validation and Facility Improvement Planning: Validate findings through facility verification and stakeholder discussions, confirm priority gaps, and develop facility-specific improvement plans alongside a public-private HCPN arrangement strategy.

Draft Report: Consolidate and analyze assessment and validation results; prepare facility-level and aggregate reports documenting key findings, readiness scores, service gaps, and recommended improvement actions.

Stakeholder Validation: Present draft findings to key stakeholders, gather feedback, and refine recommendations to ensure accuracy, relevance, and stakeholder ownership.

Final Report and Dissemination: Finalize the assessment report, improvement plan, and HCPN strategy, and disseminate findings to relevant stakeholders.

In response, partner organizations emphasized the importance of prioritization to ensure timely and practical realization of expected outcomes.

This gathering serves as a milestone in the pursuit of a comprehensive and inclusive family planning strategy, underlining the commitment of all involved to shape a brighter and healthier Philippines.

It adhered to the Leave No One Behind (LNOB) framework established by the United Nations and concentrated on five key determinants: discrimination, geography, governance, socio-economic status, and shock/fragility.

The study targeted five barangays across disaster-prone regions: Calabarzon, Eastern Visayas Region, CARAGA, and BARMM. These areas were selected based on their susceptibility to various disasters, including natural calamities and manmade crises, such as the strife in Marawi City.

The research approach comprised two integral components:

Collecting crucial background data through 23 Key Informant Interviews (KII).

Conducting in-depth Focus Group Discussions (FGD) with 107 participants to probe into their values, perceptions, beliefs, opinions, and practices concerning family planning, thereby uncovering non-health-related determinants influencing their decision-making processes.

The study’s findings revealed that women’s FP utilization experiences were significantly impacted by the following factors:

Geographic Location: The physical location of their barangays, frequently subjected to various forms of disasters, played a pivotal role in determining FP utilization patterns. This included areas affected by both natural disasters and manmade conflicts, such as the strife in Marawi City.

Socio-economic Status: The overall socio-economic standing of the communities and households was a key determinant. The study highlighted the challenges faced by women in areas characterized by limited economic resources.

Governance Quality: The effectiveness of local governance systems emerged as a critical factor. It influenced the availability and accessibility of FP services and information within these communities.

Discrimination: Experiences of discrimination had a discernible impact on FP choices. Discriminatory practices and attitudes could serve as barriers to accessing and using FP methods.

Shock and Fragility: Vulnerabilities arising from the combination of the above factors, compounded by the shock and fragility stemming from disasters, contributed significantly to women’s experiences in FP use. These shocks could exacerbate disadvantages and hardships for women of reproductive age.

While common determinants of FP use were identified across the study areas, including socio-cultural, economic, fertility, and access-related factors, there were also unique determinants specific to each region. These distinctions were primarily shaped by the ethnic, religious, and governance experiences characteristic of each area.

The study also found that women in disaster-prone areas faced multiple vulnerabilities and losses due to frequent disasters. Concerning FP use, the quality of local health and governance systems played a decisive
role in determining women’s continued inclusion or exclusion from government FP programs and services, especially during disasters.

The study’s findings underscore the importance of considering the non-health determinants of FP use when developing and implementing FP programs in disaster-prone areas. It also highlights the need for a multisectoral approach involving women, their families, government entities at both national and local levels, NGOs, and aid organizations to address the multifaceted challenges surrounding FP utilization in these areas.

The study provides the following recommendations:

Enhance access to free FP commodities, especially in the context of evacuation centers and disaster scenarios. This can be done by establishing partnerships with local government units, NGOs, and other stakeholders.

Invest in disaster-resilient FP service delivery systems. This includes training healthcare workers on FP service provision in emergency settings and ensuring the availability of essential FP supplies and equipment in evacuation centers and other temporary shelters.

Empower women to make informed decisions about their FP and reproductive health. This can be done by raising awareness about FP and reproductive rights, and promoting gender-equitable relationships.

Address the root causes of discrimination and other social determinants of health that impact FP use. This requires a multisectoral approach involving government agencies, civil society organizations, and communities.

By implementing these recommendations, we can help to ensure that all women have access to quality FP services and information, regardless of where they live or the challenges they face.

The SRH Watch project was launched in 2022 by the Philippine Society for Responsible Parenthood, Inc. (PSRP), a not-for-profit professional organization that aims to improve reproductive health and family planning in the Philippines. The project received technical and funding support from the United Nations Population Fund (UNFPA). By the end of 2022, the SRH Watch platform was deployed in the three provinces of Albay, Catanduanes, and Eastern Samar.

The SRH Watch was an innovative platform that transformed the way we tracked and enhanced Sexual and Reproductive Health (SRH) outcomes. This cutting-edge system offered real-time capabilities, serving as a crucial tool for preventing maternal deaths, a complete source of family planning data, and a vigilant monitor of contraceptive stock status. This revolutionary approach enabled decision-makers to proactively tackle emerging challenges, make informed decisions, and ensure the consistent availability of high-quality SRH services.

Real-Time Maternal Death Alerts

The SRH Watch acted as a crucial tool by providing continuous real-time tracking and analysis of maternal death rates. Decision-makers received prompt alerts and insights into changes or trends in maternal deaths, facilitating quick and targeted responses. This proactive approach saved lives by addressing the factors that caused maternal deaths.

Complete Family Planning Data Hub

As a source of family planning data, the SRH Watch gathered vital information on family planning indicators. Decision-makers had access to a wealth of data, including contraceptive use rates, unmet family planning needs, and service utilization patterns. This complete source enabled evidence-based interventions, program improvement, and addressing service delivery gaps. Real-time tracking ensured decision-makers had the most up-to-date and accurate data for swift responses.

Contraceptive Stock Status Monitoring

The SRH Watch’s real-time monitoring of contraceptive stock status integrated smoothly with the Barcode Track and Trace System (BTTS), ensuring precise and timely data updates. This integration improved early warnings and decision-making, ensuring continuous supply chains for contraceptives.

You can see the power of this innovative system in action by visiting the srh watch info websites for the three provinces where it is implemented.
On February 20, 2023, the Philippine government, in partnership with the Korea International Cooperation Agency (KOICA) and the United Nations (UN), initiated a program to combat the alarming rates of adolescent pregnancies within the country. This initiative, funded by the Republic of Korea through KOICA, is titled “Accelerating the Reduction of Adolescent Pregnancy in Southern Leyte and Samar in the Philippines.” Its principal objectives encompass improving adolescents’ access to sexual and reproductive health services and information, increasing their awareness of sexual and reproductive health rights, and fortifying leadership and governance in the realm of adolescent sexual and reproductive health (ASRH).

KOICA, in collaboration with the United Nations Population Fund (UNFPA), United Nations Children’s Fund (UNICEF), and the World Health Organization (WHO), formalized their joint program agreement scheduled to run until 2026, with the goal of making substantial strides in addressing adolescent pregnancies.

To execute some of these goals, UNFPA has enlisted the Philippine Society for Responsible Parenthood (PSRP) as one of its implementing partners. PSRP will undertake the following initiatives:

Engaging adolescents in reproductive health and gender equality through online campaigns and educational sessions by creating an adolescent community on the RH Care Info social media platform. In this community, adolescents can actively participate in campaigns and educational sessions, with a special focus on popularizing these efforts in Samar and Southern Leyte.

Enhancing the capacity of educators in Comprehensive Sexuality Education (CSE) in project sites by establishing a dedicated online community for trained teachers. This platform will facilitate the sharing of knowledge and experiences among educators implementing CSE.

Strengthening the skills of health workers in adolescent sexual and reproductive health, including family planning methods, through the creation of an online community for these professionals. This space will foster knowledge sharing and exchange of experiences among health workers engaged in ASRH.

Promoting adherence to the Performance Accountability System (PAS) among Local Government Units (LGUs) by establishing an online community for LGU officials and staff involved in the implementation of adolescent pregnancy programs. This online platform will facilitate the exchange of knowledge and experiences among these stakeholders.

With technical and financial support from UNFPA, the Philippine Society for Responsible Parenthood, Inc. (PSRP) is working with the Philippine Department of Health (DOH) to develop the Operational Guidelines on Mobile Health Units or Women’s Health on Wheels (WHoW). These guidelines will provide a comprehensive framework for implementing the mobile health clinic strategy, which aims to reach underserved and marginalized women in remote and disaster-prone areas.

The guidelines will cover a wide range of topics, including:

Technical aspects: This will include guidance on the selection of vehicles, equipment, and supplies; the development of clinical protocols and procedures; and the training of healthcare personnel.

Financial aspects: This will include guidance on the cost of setting up, operating, and maintaining WHoWs; as well as strategies for mobilizing resources from the government, private sector, and civil society.

Stakeholder roles and responsibilities: This will clarify the roles and responsibilities of the DOH, PSRP, other government agencies, civil society organizations, and communities in the implementation of the WHoW program.

The guidelines are expected to be completed by the end of this year. Once finalized, they will be disseminated to all relevant stakeholders and will serve as a valuable resource for implementing the WHoW program in a sustainable and effective manner.

The WHoW program is a significant development in the Philippine DOH efforts to improve access to healthcare for women, particularly those living in underserved and marginalized communities. By providing mobile health clinics, the WHoW program will bring essential healthcare services to women’s doorstep, regardless of their location or socioeconomic status.

In addition to providing basic healthcare services, the WHoW program will also focus on promoting women’s health and empowerment. The mobile health clinics will provide women with information and counseling on a range of topics, including reproductive health, family planning, and maternal health. The clinics will also provide women with access to essential commodities, such as sanitary napkins and contraception.

The WHoW program is a significant investment in the health and well-being of women in the Philippines. The Operational Guidelines on Mobile Health Units or Women’s Health on Wheels will play a vital role in ensuring the successful implementation of this program.

The SRH Watch platform was implemented in Albay, Catanduanes, and Eastern Samar provinces by the end of that year. Due to its promising results, the project received additional funding for 2023 and expanded to include the province of Southern Leyte. The SRH Watch is an innovative platform that changes the way we track and enhance Sexual and Reproductive Health (SRH) outcomes. This cutting-edge system offers real-time capabilities, serving as an essential tool for preventing maternal deaths, a complete source of family planning data, and a vigilant monitor of contraceptive stock status. This revolutionary approach empowers decision-makers to proactively tackle emerging challenges, make informed decisions, and ensure the consistent availability of high-quality SRH services.

Real-Time Maternal Mortality Alerts:

The SRH Watch acts as a vital tool by providing continuous real-time monitoring and analysis of maternal mortality rates.
